Studio Martial Arts – Participation Waiver, Medical Declaration & Media Consent
Participation in martial arts training, including but not limited to Brazilian Jiu-Jitsu, MMA, No-Gi Grappling, Kickboxing, Muay Thai, sparring, drilling, fitness training, and associated activities involves inherent risks of physical injury.
By participating in any classes, courses, training sessions, or events organised by Studio Martial Arts, you acknowledge and agree to the following:
1. Assumption of Risk
Martial arts training is a contact activity that involves physical exertion and controlled techniques such as throws, takedowns, joint locks, chokes, striking, grappling, and positional training.
You understand that participation carries the risk of injury including, but not limited to:
muscle strains
ligament sprains
cuts and bruises
joint injuries
broken bones
concussion
accidental injury through contact with training partners
serious injury in rare cases
You voluntarily accept these risks.
2. Voluntary Participation
Your participation in training is entirely voluntary. You accept personal responsibility for any injury, loss, or damage sustained as a result of taking part in activities at Studio Martial Arts.
3. Medical Declaration
You confirm that:
you are physically fit to participate in martial arts training
you have no medical condition that would prevent safe participation
you will inform Studio Martial Arts of any relevant medical conditions, injuries, or limitations
you will notify a coach immediately if you feel pain, discomfort, dizziness, or injury during training
You understand that Studio Martial Arts does not provide medical advice and that you are responsible for consulting a medical professional if unsure about your fitness to train.
4. Personal Responsibility
You agree to:
follow all instructions given by coaches and staff
respect training partners and act in a safe and controlled manner
train within your capabilities
stop immediately when instructed
tap or verbally submit when necessary during grappling or submission training
use protective equipment where appropriate
5. Contact Training & Sparring
You acknowledge that controlled contact is a normal part of martial arts training and may include grappling, positional sparring, and striking drills where appropriate.
6. Limitation of Liability
To the fullest extent permitted by law, Studio Martial Arts, its directors, owners, instructors, employees, contractors, and affiliates shall not be held liable for any injury, accident, loss, or damage sustained as a result of participation in training activities, except where liability cannot be excluded under UK law.
7. Personal Property
Studio Martial Arts is not responsible for the loss, theft, or damage of personal belongings brought onto the premises.
8. Photography & Video Consent
Studio Martial Arts may occasionally take photographs or video recordings during classes, events, or seminars for marketing, educational, and promotional purposes including:
website content
social media
printed materials
internal training resources
By participating, you consent to the use of your image or likeness for these purposes without compensation.
If you do not wish for your image to be used, you may notify Studio Martial Arts in writing at any time.
9. Parental / Guardian Consent (Under 18s)
If registering a participant under the age of 18, the parent or legal guardian confirms that:
they give permission for the child to participate in martial arts training
they understand the risks associated with contact sport
they confirm the child is medically fit to participate
they accept responsibility for the child’s participation
they agree to the terms of this waiver on behalf of the child
10.First Aid Consent
In the event of injury, illness, or medical emergency during participation in activities at Studio Martial Arts, you consent to qualified first aiders, coaches, or staff administering appropriate first aid treatment where necessary.
You understand that first aid may include basic medical assistance such as wound care, use of ice packs, bandaging, support of injured limbs, or other reasonable first aid measures appropriate to the situation.
Where appropriate, Studio Martial Arts staff may contact emergency services or seek further medical assistance.
You acknowledge that first aid treatment is provided in good faith to support participant welfare and that Studio Martial Arts and its staff shall not be held liable for any outcomes arising from the administration of first aid, except where liability cannot be excluded under UK law.
You confirm that it is your responsibility to inform Studio Martial Arts of any relevant medical conditions, allergies, or information that may affect emergency treatment.
11. Agreement
By signing up for classes, booking sessions, or attending training, you confirm that you have read, understood, and agreed to this waiver.
